Hayley Clark


Hayley Clark, who was appointed as the Development Lead at Carnforth Integrated Care Community in September 2022 was the speaker at our meeting on Thursday 8th June.

Linked to Morecambe Bay University Health Trust, the ICC was setup in 2014 with the intent to help support residents improve their health and wellbeing. ICC act when people are ill to try to ensure they receive the best possible joined up care and support.
The team, which has a Doctor as Clinical Lead, an Administrator, Care Coordinator and Development Lead work with Health and Care workers and Voluntary organisations.
Hayley spoke about a significant number of challenges - Access to services, Ageing population, Poverty, Health Inequalities and Transport limitations.
Hayley's role includes understanding local health issues and engaging with partners.
There is a Steering Group which meets monthly and has a guest speaker. There is also a Memory Support Group at which patients, family members and care specialists can meet to gain help and share experiences.
